Continuum of Care

The Continuum of Care (COC) program is a follow-up service for our “graduates.”  A housing specialist and registered nurse make home visits, ensuring our clients have the skills and resources they need to succeed.  

The COC helps them to facilitate their own independence and self-reliance, which fosters self-esteem and builds confidence.
